What it is, How it works

Hair analysis is increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair offers a long-term record of alcohol and drug intake by holding biomarkers within the hair's growing strands. When obtained near the scalp, hair can provide up to a 3-month detection period for alcohol and drugs. Collection is straightforward, it is resistant to tampering, and shipment is easy.

A sample size of 1.5 inches consisting of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests over 10 panels, a sample of 150mg is advisable. Use a jeweler’s scale to measure the specimen. If scalp hair is not available, a similar quantity of body hair may be used. Head hair specifically refers to scalp hair only. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is processed into the laboratory's system. It involves checking the integrity of the sealed and shipped sample, allocating a random Laboratory Accessioning Number (LAN), and completing any necessary data entry absent from an electronic chain of custody system.

Screening provides an immediate overview of drug misuse. Although cost-efficient for ruling out drug use in most samples, a positive screening needs confirmation for legal acceptance. Presumptive positive results in Screening require secondary verification.

For presumptive positives from the Screening phase, additional hair is taken from the original sample for Extraction. For confirming any positive screening outcome, GC/MS, GC/MS/MS, or LC/MS/MS techniques are employed. Before confirmation, all presumptive positives are washed as necessary. The entire lab process, from Accessioning through Confirmation, undergoes evaluation under both CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.
