For EtG, add-ons, or tests over 10 panels, a specimen amount of 150mg is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair may be utilized. Head hair refers to scalp hair alone, whereas body hair includes all other forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step involving integration of a sample into a lab’s system. This step verifies the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry that isn’t covered by an electronic chain of custody system.

Screening provides a preliminary assessment for drug abuse. It's a cost-efficient way to negate drug use in most samples, yet a positive result requires confirmation for legal admissibility. Samples that are presumptively positive in Screening require follow-up confirmation.

If a sample tests presumptively positive at the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. At this juncture, drugs are extracted from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening notably challenging.

GC/MS, GC/MS/MS, or LC/MS/MS techniques confirm any positive Screening outcomes. Presumptive positive samples undergo washing before confirmation as necessary. The full laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
