A sample of approximately 1.5 inches consisting of about 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, supplemental tests, and/or tests beyond 10 panels, 150mg of the specimen is advised. It is recommended to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent volume of body hair may be obtained. References to head hair imply scalp hair exclusively. Body hair denotes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a laboratory’s system. This process includes verifying the sample’s seal and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary swift assessment for drugs of abuse. Although Screening is an economical method to eliminate drug usage in most samples, a positive screen must be confirmed to be permissible in court. Any samples that are tentatively positive in Screening need a subsequent confirmation.

If a sample yields a presumptive positive during the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to conduct.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
