Typically, 1.5 to 2 inches of hair is required, either from the head or body. Hair analyses can be conducted for substances, alcohol/ETG, or other tailored testing requests.

In recent times, drug screening methods have increasingly adopted hair follicle tests. More employers, judicial systems, and substance abuse experts now prioritize hair-based evaluations over conventional urine tests. Hair follical screenings are favored by employers adhering to strict no-drug policies, courts, and probation officers. They benefit from an extensive detection window, usually up to 90 days, administered under supervision, making them hard to tamper with. Nevertheless, urine screenings still offer the highest accuracy for determining substance use within the past five days.

What it is, How it works

Hair analysis has become recognized as an effective instrument for spotting alcohol and drug usage. Hair offers an extended history of drugs and alcohol by preserving biomarkers within the fibers of the expanding hair shaft. When gathered near the scalp, hair can give up to around a 3-month period of detection for substances like alcohol and drugs. Hair is straightforward to collect, somewhat challenging to adulterate, and simple to transport.

A sample 1.5 inches in length consisting of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg, which is the preferred amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is preferred. We suggest using a jeweler's scale to weigh the sample. In cases where scalp hair is not available, an equivalent amount of body hair can be used. References to head hair pertain exclusively to scalp hair, while body hair encompasses all other hair sections (facial, axillary, etc.).

Process Overview

There are four principal steps associated with lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into the lab's system. This step includes checking if the specimen was securely sealed and correctly transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody.

Screening is an initial rapid check for drugs of abuse. Although Screening serves as a cost-efficient technique to dismiss drug use across numerous samples, a positive outcome must be substantiated for judicial acceptability. Samples showing presumptive positivity demand secondary confirmation.

If a specimen is deemed presumptively positive at the Screening stage, further hair is drawn from the initial batch and readied for Extraction. During this phase, drugs are drawn from hair at a much lower concentration than other methodologies (e.g., urine or oral fluid), which makes hair drug analysis the most challenging methodology to execute.

Confirmation on any positive screening results is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation as needed. The whole lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Benefits of hair drug testing:

  • Extended detection period: Hair tests can find drug presence for up to 90 days, as compared to urine tests that have a shorter detection span.
  • Hard to mislead: It is notably difficult to falsify a hair drug test, making outcomes more credible.
  • Illustrates historical usage: It can demonstrate a pattern of drug use across time, not solely recent usage.

Drawbacks:

  • Inability to detect immediate use: It requires about 5-7 days for drugs to manifest in hair.
  • Expense: Hair tests generally cost more than alternative drug testing methodologies.
  • Result variability: Elements like hair color and personal hair growth differences can affect drug metabolite levels in hair.

Note: Despite being often termed "hair follicle tests", the actual examination is conducted on the hair strand and not the hair follicle beneath the scalp.
