When gathered near the scalp, hair can give up to around a 3-month period of detection for substances like alcohol and drugs. Hair is straightforward to collect, somewhat challenging to adulterate, and simple to transport.

A sample 1.5 inches in length consisting of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg, which is the preferred amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is preferred. We suggest using a jeweler's scale to weigh the sample. In cases where scalp hair is not available, an equivalent amount of body hair can be used. References to head hair pertain exclusively to scalp hair, while body hair encompasses all other hair sections (facial, axillary, etc.).

Process Overview

There are four principal steps associated with lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into the lab's system. This step includes checking if the specimen was securely sealed and correctly transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody.

Screening is an initial rapid check for drugs of abuse. Although Screening serves as a cost-efficient technique to dismiss drug use across numerous samples, a positive outcome must be substantiated for judicial acceptability. Samples showing presumptive positivity demand secondary confirmation.

If a specimen is deemed presumptively positive at the Screening stage, further hair is drawn from the initial batch and readied for Extraction. During this phase, drugs are drawn from hair at a much lower concentration than other methodologies (e.g., urine or oral fluid), which makes hair drug analysis the most challenging methodology to execute.

Confirmation on any positive screening results is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation as needed. The whole lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
