The laboratory processing of drug test outcomes features four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Samples yielding presumptive positive results during Screening must undergo additional confirmation.

For instances where a sample tests positive during Screening, additional hair is extracted from the specimen for Extraction. This phase involves extracting substances from hair at significantly lesser concentrations than seen in other techniques (like urine or oral fluids), accentuating the complexity of hair drug screening methodologies.

Verification of any positive results from Screening is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes undergo washing before confirmation as necessary. The complete laboratory workflow, encompassing Accessioning to Confirmation, is audited per both the CAP (College of American Pathologists) Hair designation and the accreditation according to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
