It's straightforward to gather, relatively tough to tamper with, and simple to send.

A 1.5-inch segment of roughly 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample size for testing and validation. For EtG, advanced screening, or tests exceeding 10 panels, 150mg of hair is suggested. It's advisable to weigh the hair using a jeweler's scale. If head hair is unobtainable, a comparable quantity of body hair can be used. Head hair strictly refers to scalp hair. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome typically involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is integrated into a laboratory’s database. It involves ensuring the sample was securely sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any further data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of misuse. Although this is an economical way to eliminate drug presence in most samples, a positive result in Screening necessitates verification for judicial acceptability. Any specimens presumably positive in Screening mandate a subsequent confirmation.

If a sample registers as presumably positive during Screening, additional hair is drawn from the original sample and readied for Extraction. In this phase, substances are isolated from hair at significantly lower concentrations than alternative methods (for instance, urine or saliva), underscoring why hair drug testing ranks as the most complex to execute.

Using GC/MS, GC/MS/MS, or LC/MS/MS, confirmation of positive findings from Screening is undertaken. All presumptive positive samples undergo a washing process prior to confirmation if required. The full lab workflow from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair guideline and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
