A hair sample measuring 1.5 inches and consisting of about 200 strands (comparable to a #2 pencil) near the scalp yields 100mg, the preferred quantity for screening and validation. For EtG add-ons and/or tests beyond 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. In cases where scalp hair is lacking, an equivalent quantity of body hair can be collected. 'Head hair' refers exclusively to scalp hair, whereas 'body hair' includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the sample’s initial processing into the lab’s system. This step checks that the sample was correctly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry omitted by an electronic chain of custody system.

Screening provides an initial rapid check for drug use. Although cost-effective for eliminating drug use in most samples, a positive screen requires confirmation to be valid in legal situations. Any samples presumptively positive in Screening necessitate further confirmation.

If a sample is presumptively positive in the Screening process, additional hair is extracted from the primary specimen to prepare for Extraction. This stage involves extracting drugs from the hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging to execute.

Confirmation of any positive screening outcome occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive undergo washing before confirmation as necessary. The full lab process from Accessioning to Confirmation complies with the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
