A sample 1.5 inches in length, consisting of about 200 hair strands (comparable to a #2 pencil's diameter) closest to the scalp, provides 100mg of hair, which is ideal for screening and confirmation. For EtG tests, additional tests, and/or screens with more than 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not accessible, an equal quantity of body hair may be obtained. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s system. This process includes confirming that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and filling in any further data not supplied by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While it offers a cost-efficient approach to exclude drug usage in most samples, a positive screening result must be confirmed to be valid for court proceedings. Samples yielding a presumptive positive in Screening necessitate a follow-up confirmation.

If a sample receives a presumptive positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are retrieved from hair, present at a considerably lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

GC/MS, GC/MS/MS or LC/MS/MS are used to confirm any positive screening findings. All samples with a presumptive positive result are cleansed before confirmation, as required. The complete lab process from Accessioning through Confirmation is subject to review under both the CAP (College of American Pathologists) Hair classification as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
