The advantage lies in its 90-day detection window, collecting samples under observation, nearly eliminating cheating. Yet, urine tests remain best for identifying recent consumption within 5 days.

What it is, How it works

Utilizing hair analysis is increasingly viewed as an effective means for identifying drug and alcohol consumption. The hair retains biomarker evidence within its strands as it grows, offering an extended overview of substance use. When samples are taken directly from the scalp, they can capture a detection period of up to around 90 days for substances. Hair can be readily collected, has limited susceptibility to tampering, and is straightforward to mail.

A sample of 1.5 inches consisting of approximately 200 hair strands (around the thickness of a #2 pencil) close to the scalp yields around 100mg of hair, which is considered an optimal amount for both screening and verification procedures. For EtG enhancements and/or testing beyond 10 panels, a sample of 150mg is advocated. We advise measuring the sample with a jeweler’s scale. Should scalp hair not be available, an equivalent quantity of body hair can be used. When discussing head hair, we exclusively mean scalp hair. Body hair includes all other hair varieties (facial, axillary, etc.).

Process Overview

Lab processing of drug test samples follows four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves introducing the sample to the lab system initially. This process confirms that the sample was securely sealed and correctly dispatched, allocates a unique LAN (Laboratory Accessioning Number), and finalizes any incomplete data entry not handled by an electronic tracking system.

Screening is an initial rapid assessment for drugs. While Screening is a budget-friendly method for excluding drug usage in most samples, confirmation is needed for any court admissible positive results. Any sample that shows a tentative positive during Screening must be further confirmed.

For samples showing a presumptive positive in Screening, additional hair is taken from the original sample and prepared for Extraction. Drugs are extracted from hair at a lower concentration than other testing methods (e.g., urine or oral fluid), rendering hair testing the most challenging methodology to conduct.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that potentially test positive are cleansed before confirmation as warranted. Every aspect of the laboratory procedure, from Accessioning through to Confirmation, is evaluated under the guidelines of both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
