What it is, How it works

Hair testing is regarded as an effective method for identifying usage of alcohol and drugs. The strands capture biomarkers in their fibers, providing an extended overview of substance use. When taken near the scalp, they offer a detection window of up to about three months for both alcohol and drugs. Hair is easy to gather, relatively resistant to tampering, and straightforward to ship.

Taking a 1.5-inch segment of roughly 200 hair strands (comparable in size to a #2 pencil) nearest the scalp provides 100mg of hair, the standard sample size for tests and confirmations. For EtG, extra tests, and/or panels beyond 10, 150mg of specimen is advised. We suggest checking the weight of the specimen on a jeweler’s scale. If scalp hair is not an option, an equivalent quantity of body hair can be gathered. By head hair, we mean only scalp hair, whereas body hair encompasses all other types, such as facial and axillary.

Process Overview

The primary steps in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a sample into a laboratory’s framework. This step checks if the sample was sealed and transported appropriately, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not managed by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Although Screening is affordable to eliminate drug use in most samples, a positive screen needs further confirmation before being eligible for legal proceedings. Samples presumptively positive in Screening must undergo a secondary validation.

If a sample appears positive in Screening, more specimen is extracted and set for Extraction. Here, drugs are removed from hair at lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging process to execute.

Confirming any positive screen result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleaned as required. The full lab procedure from Accessioning through Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair guidelines and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
