A 1.5-inch segment comprising approximately 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp is needed to acquire 100mg of hair, which constitutes the optimal sample for both screening and verification. For EtG enhancements and/or testing exceeding 10 panels, a 150mg sample is advised. We suggest measuring the sample on a jeweler’s scale. Should scalp hair be unavailable, an equivalent volume of body hair can be utilized. In referencing head hair, we specify scalp hair alone. Body hair signifies all other forms (facial, axillary, etc.).

Process Overview

The principal components of the lab processing routine for drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the preliminary sample processing into a laboratory system occurs. This process encompasses confirming proper sealing and shipping of the sample, issuing a random LAN (Laboratory Accessioning Number), and concluding any further data input absent in an electronic chain of custody interface.

Screening entails a rapid preliminary test for illicit substances. Although Screening offers a cost-efficient method for excluding drug usage in most instances, a positive screen inevitably necessitates confirmation to ensure legal admissibility. Samples that are initially positive in Screening require a follow-up verification.

If initially positive at the Screening phase, additional hair is extracted from the preliminary sample and prepped for Extraction. During this phase, drugs are separated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which accounts for hair drug screening being the most intricate methodology to execute.

Confirmation of any positive results from Screening is conducted utilizing GC/MS, GC/MS/MS or LC/MS/MS technologies. Prior to confirmation, all presumptive positive samples undergo cleansing as required. Each stage from Accessioning to Confirmation is scrutinized under the dual guidance of the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
