To include EtG, additional tests, and/ or panels over 10, the recommended sample size increases to 150mg. Using a jeweler’s scale for weighing the specimen is advisable. In absence of scalp hair, an equivalent quantity of body hair is an alternative. References to head hair are specific to scalp hair, whereas body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a laboratory’s operations. It entails ensuring the sample is appropriately sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any further data requirements absent from an electronic chain of custody system.

Screening serves as a preliminary check for drug abuse. While Screening is an affordable method for excluding drug use in most samples, positive results from screening need confirmation to be valid in legal proceedings. Samples indicating presumptive positivity in Screening require further confirmation.

If a sample appears positive during Screening, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are separated from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluids), making hair drug screening the most challenging methodology to execute.

Confirmation of a positive screening result is handled through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every presumptive positive sample undergoes washing prior to necessary confirmation. The complete laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
