Taking a 1.5-inch segment consisting of roughly 200 hair strands (roughly equating to a #2 pencil's width) closest to the scalp yields 100mg of hair, which constitutes an optimum sample for both screening and confirmation steps. For EtG, supplemental tests, and/or screenings exceeding 10 panels, a specimen weight of 150mg is advisable. Utilizing a jeweler's scale to gauge the specimen's weight is suggested. If scalp hair isn't an option, a comparable quantity of body hair is an acceptable alternative. References to head hair pertain exclusively to scalp hair, while body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory procedures for processing drug test results involve four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a lab's system. This step confirms the sample remains sealed and shipped appropriately, allocates a random LAN (Laboratory Accessioning Number), and finalizes any other data entry not managed by an electronic chain of custody system.

Screening consists of a preliminary swift inspection for substance misuse. Although Screening is an economical method to exclude drug usage in most cases, any positive results necessitate validation to be valid in legal contexts. Presumptively positive samples in the Screening phase require further confirmation.

Should a sample appear presumptively positive during Screening, additional hair is extracted from the original specimen for Extraction. At this juncture, drugs are extracted from the hair at a significantly lower concentration than alternative methods (e.g., urine or saliva), which underscores why hair drug testing is among the more intricate methodologies to execute.

Confirmation of any screening that yields a positive result is undertaken using methodologies such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo a cleansing process before confirmation where necessary. From Accessioning through to Confirmation, the entire laboratory procedure is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
