What it is, How it works

When samples are taken close to the scalp, they can offer an approximate 3-month window for detecting alcohol and drug use. Hair collection is straightforward, somewhat resistant to tampering, and simple to transport.

A sample measuring 1.5 inches with approximately 200 hair strands (roughly the thickness of a #2 pencil) nearest the scalp yields 100mg, an optimal amount for screening and verification. For EtG tests, additional testing, and/or tests exceeding 10 panels, a 150mg sample is advised. The specimen should be weighed using a jeweler’s scale. If scalp hair is unavailable, equivalent body hair can be collected. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the initial processing of a sample into a lab’s system, which involves ensuring that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is an initial rapid check for substances of abuse. While Screening presents a cost-effective way to dismiss drug usage for most samples, a positive screen needs confirmation to be valid in court. Samples showing a presumptive positive in Screening necessitate further confirmation.

If a sample tests presumptively positive in the Screening phase, additional hair is retrieved from the original specimen and readied for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations than in other testing methods (e.g., urine or oral fluid), which is why hair drug screening is one of the most challenging methodologies to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation as necessary. The full lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
