What it is, How it works

Hair analysis has emerged as an effective method for identifying substance and alcohol intake. When hair is collected near the scalp, it can offer a detection period of up to 90 days for substances and alcohol. The collection process is straightforward, with minimal chances of tampering, and is easy to transport.

A sample measuring 1.5 inches which consists of approximately 200 hair strands (similar to a #2 pencil's width) closest to the scalp will provide 100mg of hair, the perfect amount for assessment and validation. For EtG, additional testing, and/or panels exceeding 10, 150mg of hair is advised. It is suggested to use a jeweler’s scale for weighing the sample. If scalp hair isn't an option, a comparable quantity of body hair can be used. In terms of head hair, we mean only scalp hair. Body hair includes all other types, such as facial or axillary hair.

Process Overview

Laboratory analysis of a drug test involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is integrated into a lab's system. This consists of confirming that the sample was appropriately sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional information input not provided by an electronic custody system.

Screening is an initial, rapid assessment for substance misuse. Although Screening is a cost-efficient method to exclude substance abuse in most cases, a positive result needs confirmation to fulfill legal standards. Any samples that test positive during Screening necessitate further confirmation.

When a sample tests presumptively positive during Screening, further hair is extracted from the initial sample and readied for Extraction. During this step, substances are isolated from hair at a notably lower concentration compared to other procedures (e.g., urine or oral fluid), making hair drug screening one of the most challenging methods to execute.

Confirmation of any positive results from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that test presumptive positive are cleansed before confirmation if necessary. The complete lab procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
