What it is, How it works

Hair analysis is perceived as a highly effective means for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and various drugs by embedding biomarkers within the fibers of the emerging hair shaft. When collected near the scalp, hair can offer a detection period of up to three months for alcohol and different substances. Hair is straightforward to collect, moderately challenging to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately equivalent in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal specimen size for evaluation and verification. For EtG, supplements, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the specimen's weight using a jeweler’s scale. If scalp hair is unobtainable, a comparable volume of body hair may be retrieved. Regarding head hair, we specifically mean scalp hair exclusively. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for a drug test result comprises four critical phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a laboratory's framework. This process includes confirming that the sample was correctly sealed and delivered, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data input not furnished by an electronic chain of custody system.

Screening involves a preliminary rapid inspection for substance misuse. Despite being a cost-efficient approach to rule out drug consumption in a majority of samples, a positive screening outcome must be confirmed for it to be legally admissible. Any samples with a tentative positive Screening necessitate subsequent verification.

Should a sample indicate a presumptive positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this process, drugs are retrieved from hair at a considerably diminished concentration compared to other techniques (such as urine or oral fluids), making hair drug examination the most arduous method to execute.

Verification of any positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The complete laboratory sequence, from Accessioning to Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standard accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
