The term "head hair" specifically means scalp hair, while "body hair" encompasses all other hair forms (such as facial and axillary).

Process Overview

The processing of a drug test result in a lab primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into the lab’s system. This step involves ensuring the sample is properly sealed and shipped, assigning a Laboratory Accessioning Number (LAN) at random, and conducting any supplemental data input that is not part of an electronic chain of custody system.

Screening is a preliminary rapid test for drug detection. It's an economical method to eliminate potential drug use for most samples, but a positive result must be validated to be used in legal proceedings. Any sample that tests presumptively positive during Screening necessitates a further confirmatory test.

If a sample tests presumptively positive in Screening, additional hair is extracted from the initial sample for the Extraction phase. In this step, drugs are removed from the hair at significantly lower concentrations than with other testing methods (e.g., urine or oral fluid), making hair drug screening a particularly challenging method to conduct.

Confirmation of any positive screen result is done via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that presumptively test positive are cleansed before confirmation as needed. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
