When obtained close to the scalp, hair enables detection of alcohol and other drugs over a period of up to roughly three months. Collecting hair is straightforward, relatively hard to tamper with, and convenient to transport.

A 1.5-inch portion consisting of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or panels exceeding ten, a 150mg sample is advised. It is recommended to weigh the sample on a jeweler's scale. If scalp hair is not available, an equivalent quantity of body hair may be used. "Head hair" refers specifically to scalp hair. "Body hair" includes all other types (such as facial or axillary hair).

Process Overview

The four primary phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a lab’s system. This comprises checking that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry missing from an electronic chain of custody system.

Screening comprises an initial quick test for drugs of abuse. Although Screening is an economical method to eliminate drug use in the majority of samples, a positive screen necessitates confirmation to be permissible in court. Any samples appearing positive in Screening must undergo a secondary confirmation.

If a sample appears positive during the Screening phase, additional hair is taken from the original specimen and readied for Extraction. During this phase, drugs are removed from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug testing the most challenging methodology to perform.

Confirmation of any positive screen outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as required. The entire lab process, from Accessioning to Confirmation, is examined under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
