For EtG and broader tests involving more than 10 panels, a sample of 150mg is ideal. Using a jeweler’s scale for accurate measurement is advisable. If scalp hair is unavailable, an equal amount of body hair is suitable. By 'head hair,' we specifically mean scalp hair, while 'body hair' includes all other types such as facial and axillary hair.

Process Overview

The laboratory process for a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the formal entry of a sample into the lab system, confirming the sample was correctly sealed and shipped, assigning a Laboratory Accessioning Number (LAN) at random, and completing additional data entries as needed when not provided by an electronic chain of custody.

Screening provides a preliminary assessment for drug presence. Though economical for ruling out drug use in many samples, a positive Screening result requires confirmation to be legally defensible. Samples that test positive during Screening need a further confirmatory step.

When a sample tests positive in Screening, additional hair is taken from the initial sample for the Extraction phase. Drugs are separated from hair in this stage at significantly lower concentrations than other methods (such as urine or oral fluids), which makes hair drug testing a complicated process.

Any positive screenings undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Presumptive positives are cleaned before confirmation. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
