A sample consisting of 1.5 inches of approximately 200 hair strands (comparable to the diameter of a #2 pencil) near the scalp collects 100mg of hair, which is optimal for both screening and validation. For EtG, supplementary tests, or analyses exceeding 10 panels, a specimen size of 150mg is advisable. Using a jeweler’s scale to weigh the specimen is suggested. In the absence of scalp hair, a corresponding quantity of body hair may be used. When we mention head hair, it specifically refers to scalp hair, while body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The primary phases in the laboratory evaluation of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary processing of a sample into the lab's system. This stage confirms that the sample was properly sealed and shipped, generates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from an electronic chain of custody system.

Screening executes an preliminary swift assessment for drug abuse. While Screening is a cost-efficient method to invalidate drug usage in the bulk of samples, any positive screen mandates confirmation to be valid in a legal setting. Samples testing presumptively positive during Screening do require a subsequent confirmation.

Should a specimen indicate a presumptive positive result in the Screening phase, additional hair is drawn from the initial sample and prepped for Extraction. During Extraction, drugs are extracted from hair at concentrations significantly lower than in alternative methods (such as urine or oral fluid), underscoring why hair drug testing is the most intricate method to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed ahead of confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, is assessed in accordance with both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
