If scalp hair is unavailable, a similar amount of body hair may be used. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The primary phases in laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into the lab’s system. It ensures the sample was properly sealed and shipped, allocates a unique LAN (Laboratory Accessioning Number), and completes any required manual data entry, supplementing an electronic chain of custody.

Screening involves a preliminary assessment to detect drugs. Although cost-effective in excluding drug usage in most samples, a positive result in Screening mandates further confirmation to be valid in legal proceedings. Any presumptive positives in Screening necessitate additional verification.

Should Screening indicate a presumptive positive, more hair is extracted from the initial collection for further processing in Extraction. Here, drugs are extracted from hair at much lower concentrations compared to other testing methods such as urine or oral fluid, making hair testing particularly challenging.

Confirmation of any positive results is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are pre-washed before Confirmation if required. The complete process from Accessioning to Confirmation is evaluated following the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
