What it is, How it works

Hair testing is recognized as a potent method for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and drugs by embedding biomarkers within the threads of the emerging hair. When sampled close to the scalp, hair can detect alcohol and other drugs for an approximate duration of 3 months. Collection of hair is straightforward, slightly tamper-resistant, and simple to transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable to the thickness of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal sample size for testing and validation. For EtG, extra tests, and/or testing beyond 10 panels, 150mg of hair is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is not available, a similar quantity of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

There are four key stages in the laboratory analysis of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a laboratory's database. This consists of confirming that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data tasks not handled by an electronic chain of custody system.

Screening executes a preliminary assessment for illicit substances. Although Screening is an economical approach to exclude drug consumption in most samples, a positive screen must be confirmed for court acceptance. Any samples flagged as suspiciously positive in Screening necessitate a follow-up confirmation.

Should a sample appear positive in Screening, additional hair is extracted from the original sample and readied for Extraction. At this juncture, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of positive Screening outcomes is done using GC/MS, GC/MS/MS, or LC/MS/MS. All tentative positive samples undergo washing before confirmation as necessary. The comprehensive lab procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
