For EtG tests, additional tests, and/or tests beyond 10 panels, a specimen weighing 150mg is advisable. It's suggested to weigh the sample using a jeweler's scale. In cases where scalp hair is unattainable, a similar amount of body hair may be used. When discussing head hair, it pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The process of laboratory testing for drug results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a laboratory setting. This phase includes verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid check for drugs of abuse. Although Screening is a budget-friendly method to eliminate drug usage in the majority of samples, a positive screening outcome must be validated to be permissible in court. Samples with an initial positive result in Screening necessitate subsequent confirmation.

Should a sample initially test positive in the Screening stage, more hair is extracted from the original specimen and prepared for the Extraction phase. In this phase, drugs are extracted from hair at a notably lower concentration than with other methods (such as urine or oral fluid), highlighting why hair drug screening presents the most complexity in execution.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples flagged as presumptively positive are cleansed before confirmation as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
