A 1.5-inch segment comprising about 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for both screening and verification. For EtG, additional tests and/or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler's scale to weigh the specimen. If scalp hair is not accessible, a comparable quantity of body hair may be gathered. When referring to head hair, we mean scalp hair exclusively. Body hair indicates all other types of hair (facial, axillary, etc.).

Process Overview

The primary steps in laboratory processing for a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into the laboratory's system. This encompasses confirming that the sample was appropriately sealed and transported, generating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data input not supplied by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient method to eliminate drug usage from the bulk of samples, a positive screen requires confirmation to hold up in a court of law. Any samples that show presumptive positives in Screening necessitate further confirmation.

If a sample shows presumptive positives during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are removed from hair at a significantly reduced concentration compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if needed. The entire lab process from Accessioning to Confirmation is evaluated according to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
