A sample size of 1.5 inches composed of roughly 200 strands (similar to a #2 pencil thickness) from near the scalp yields around 100mg of hair, which is deemed optimal for both screening and confirmation. For EtG add-ons and/or tests exceeding 10 panels, an increased sample of 150mg is advisable. It's recommended to measure the specimen using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair is acceptable. "Head hair" specifically refers to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial stage of integrating a sample into the laboratory system, involving verification of proper sealing and shipping, assignment of a random LAN (Laboratory Accessioning Number), and completion of any missing data entry that an electronic chain of custody system doesn’t cover.

Screening conducts a preliminary analysis for signs of drug abuse. While it provides an economical method for excluding drug use in most cases, any positive results from this stage require confirmation to be admissible in court. All samples that test presumably positive in Screening necessitate further confirmation.

Should a sample yield a presumptive positive result during the Screening phase, additional hair is separated from the original sample and readied for Extraction. In this phase, drugs are extracted from hair at substantially lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing particularly challenging to execute.

Confirmation of any positive result during Screening employs techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.
