What it is, How it works

Hair analysis has gained recognition as an effective means for identifying substance use. Hair serves as a long-term record of alcohol and drug use by capturing biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer a detection window of up to approximately three months for alcohol and other drugs. Hair is easy to collect, challenging to tamper with, and simple to transport.

A sample of 1.5 inches consisting of around 200 hair strands (similar in size to a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample for testing and confirmation. For EtG, additional tests, and/or screenings beyond 10 panels, a sample size of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent amount of body hair may be obtained. When mentioning head hair, we refer specifically to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into a laboratory’s system. This includes ensuring the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry that isn’t provided by an electronic chain of custody system.

Screening is a preliminary evaluation for the presence of drugs of abuse. While Screening is a cost-efficient method to eliminate drug use for most samples, a positive screen must be verified to be valid in legal settings. Samples that are presumptively positive in Screening require a secondary confirmation.

If a sample tests presumptively positive during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are isolated from hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
