Hair can offer a comprehensive history of alcohol and drug use by capturing biomarkers within the strands as they grow. When hair is cut near the scalp, it can reveal up to a 3-month period of alcohol and other drug detection. Hair collection is straightforward, hard to tamper with, and easy to transport.

A sample of 1.5 inches containing about 200 hairs (similar to a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advised. It's best to weigh the sample using a jeweler’s scale. If scalp hair can't be obtained, an equivalent volume of body hair can be taken. "Head hair" refers specifically to scalp hair only. "Body hair" encompasses all other types such as facial and axillary hair.

Process Overview

The process of laboratory evaluation for drug testing consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary phase of integrating a sample into a lab's framework. This process includes checking the sample seal and shipment correctness, assigning it a random LAN (Laboratory Accessioning Number), and inputting any extra details not available in an electronic chain of custody record.

Screening provides an initial drug abuse assessment. Screening offers a cost-efficient means to exclude drug use for most samples, though any positive outcome must be verified for legal acceptability. Samples with a presumptive positive result in Screening necessitate further confirmation.

For samples that test presumptively positive during Screening, additional hair is extracted from the primary sample and readied for Extraction. In this phase, the drugs are isolated from hair at a notably lower concentration than other methods (e.g., urine or saliva), making hair drug analysis the most challenging technique.

To confirm any positive screening findings, GC/MS, GC/MS/MS or LC/MS/MS techniques are employed. All samples indicating presumptive positives undergo cleansing before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, is overseen following both CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
