A sample of 1.5 inches, consisting of around 200 hair strands (comparable to the diameter of a #2 pencil) near the scalp, yields 100mg of hair, which is the optimal quantity for analysis and validation. For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It's suggested to measure the sample using a jeweler’s scale. If head hair is absent, a similar amount of body hair can be gathered. When discussing head hair, we refer strictly to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four pivotal stages in the lab handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of integrating a sample into a laboratory’s protocol. This step confirms that the sample has been adequately sealed and sent, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any remaining data input not supplied by an electronic chain of custody framework.

Screening is a preliminary swift assessment for substance abuse. Although Screening is an economical approach to eliminate drug consumption in most samples, a positive screen necessitates substantiation for judicial acceptability. Any samples deemed presumptively positive during Screening must undergo further confirmation.

Should a sample show a presumptive positive in the Screening phase, additional hair is taken from the original specimen to prepare for Extraction. Here, drugs are removed from hair at a significantly lower concentration compared to other techniques (such as urine or oral fluid), highlighting the complexity of conducting hair drug screening.

Verification of any positive screening outcomes is performed utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as required. The complete lab process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair designation and is certified under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
