In the absence of scalp hair, a corresponding quantity of body hair may be used. 'Head hair' specifically refers to scalp hair, whereas 'body hair' encompasses all other types (such as facial or axillary hair).

Process Summary

The laboratory analysis of drug test samples involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of entering the sample into the laboratory's records. It involves ensuring the sample was securely sealed and correctly sent, assigning a random Laboratory Accessioning Number (LAN), and carrying out any necessary data entry not covered by the electronic chain of custody.

Screening is the initial rapid assessment for drug presence. While Screening offers an affordable method for excluding drug use in most samples, a positive result from Screening requires confirmation for legal validity. Any samples presumed positive in Screening necessitate secondary verification.

Should a sample appear positive in Screening, additional hair is extracted from the original sample batch for the Extraction phase. In this step, drugs are isolated from hair in lower concentrations than other methods (e.g., urine or saliva), which contributes to the complexity of performing hair drug analysis.

Confirmation of any positive results found in Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive undergo washing before Confirmation as necessary. The entire lab process from Accessioning to Confirmation is subject to oversight under both the CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
