A 1.5-inch segment of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will provide 100mg of hair, the ideal sample size for both screening and confirmation. For EtG, add-ons, and/or assessments above 10 panels, 150mg of the sample is advised. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be collected. When specifying head hair, it pertains solely to scalp hair. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The four main processes in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample within a laboratory’s system. This process verifies that the sample was securely sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finishes any data entry not provided by an electronic chain of custody system.

Screening offers an initial swift assessment for drugs of abuse. While Screening is cost-effective for ruling out drug use in most samples, a positive screen must be confirmed to be court-admissible. Samples that are presumptively positive during Screening require further confirmation.

If a sample is presumptively positive at the Screening stage, additional hair is taken from the initial specimen for Extraction. In this stage, drugs are extracted from hair at a much lower concentration than alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
