What it is, How it works

When sampled close to the scalp, hair can offer approximately a three-month detection range for both alcohol and drugs. Hair collection is straightforward, relatively tamper-proof, and simple to transport.

A 1.5-inch segment, consisting of approximately 200 hairs (similar in thickness to a #2 pencil), taken nearest to the scalp, yields around 100mg of hair, which is the ideal quantity for analysis and confirmation. For EtG, supplementary tests, and/or panels beyond 10, 150mg of hair is suggested. It is advisable to measure the sample's weight using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair may be used. Head hair refers exclusively to scalp hair, whereas body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's system. This process verifies the sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data input not covered by an electronic chain of custody system.

Screening conducts a preliminary rapid check for drugs. Although Screening is a cost-effective approach to exclude drug use in most samples, a positive finding requires confirmation for legal admissibility. Samples that are presumed positive in Screening necessitate additional verification.

For samples presumed positive during Screening, more hair is drawn from the original sample for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than in other methods (e.g., urine or saliva), which makes hair drug testing the most complex method to execute.

Confirmation of any positive result from Screening is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
