When collected near the scalp, hair specimens can offer up to a three-month detection period for alcohol and drugs. It is straightforward to collect hair, somewhat hard to tamper with, and convenient to ship.

A sample measuring 1.5 inches, approximately 200 hair strands (similar to the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of specimen is advised. We suggest evaluating the specimen’s weight using a jeweler’s scale. If scalp hair is not accessible, a comparable amount of body hair may be acquired. When discussing head hair, we are exclusively referring to scalp hair. Body hair encompasses all other hair types, such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within a laboratory’s system. This process includes checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional data entry not provided by an electronic chain of custody system.

Screening is an initial swift analysis for drugs of abuse. Although it’s a cost-effective approach to prove drug absence in most samples, positive screening results must be verified to be admissible in legal proceedings. Presumptive positive samples in Screening mandate a secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is drawn from the sample for Extraction. In this stage, drugs are extricated from hair at substantially lower concentrations than other methods (e.g. urine or oral fluid), which explains why hair drug testing is the toughest method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS are used for confirming any positive screening results. All presumptive positive samples undergo washing before confirmation whenever necessary. The entire process from Accessioning to Confirmation undergoes review following both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
