A 1.5-inch section of approximately 200 hair strands (roughly the thickness of a #2 pencil) nearest the scalp yields 100mg of hair, which is the ideal amount for testing and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the sample is suggested. We propose measuring the sample with a jeweler’s scale. Should scalp hair be unavailable, a similar volume of body hair can be used. The term head hair refers solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four fundamental steps in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a laboratory’s framework. This process includes ensuring the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry not covered by an electronic chain of custody system.

Screening involves a swift preliminary assessment for drugs. Although Screening is a cost-efficient method to exclude drug usage in most samples, a positive result requires confirmation to be court-acceptable. Any samples that test presumptively positive in Screening need a secondary confirmation.

If a sample tests positive in the Screening phase, additional hair is drawn from the original sample and readied for Extraction. In this phase, drugs are extracted at significantly lower concentrations than in other methodologies (such as urine or oral fluids), which is why hair drug analysis is the most challenging methodology to conduct.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All samples that are presumptive positives undergo a cleansing prior to confirmation as necessary. The lab process from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
