What it is, How it works

When collected near the scalp, hair can offer a detection period of up to around three months for alcohol and various drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to send.

A 1.5-inch section of approximately 200 strands closest to the scalp will yield 100mg of hair, the optimal quantity for analysis and confirmation. For EtG, additional tests, and/or panels above 10, a sample size of 150mg is suggested. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, the equivalent amount of body hair may be gathered. When discussing head hair, it pertains solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to introducing a sample into the lab's system, ensuring the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening serves as an initial quick scan for drugs. Despite its cost-effectiveness for excluding drug use in most cases, a positive screening outcome must be confirmed for court admissibility. Samples testing presumptively positive during Screening require secondary confirmation.

Should a sample show presumptive positivity at the Screening phase, additional hair is taken from the initial sample for the Extraction stage. Here, drugs are isolated from hair at a lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of any positive Screening result occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo washing prior to necessary confirmation. The full lab process from Accessioning to Confirmation undergoes review in line with both CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
