What it is, How it works

Hair analysis has emerged as an influential means for identifying alcohol and drug consumption. Hair serves as a long-term record of alcohol and drug use by embedding biomarkers in the fibers of the hair as it grows. When taken from near the scalp, hair can provide roughly a 3-month detection period for drugs and alcohol. It is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample for both screening and validation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to measure the sample. If scalp hair is unavailable, a similar amount of body hair can be collected. When mentioning head hair, we are specifically referring to scalp hair. Body hair refers to other hair types (facial, axillary, etc.).

Process Outline

The four primary steps in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab's system. This includes confirming that the sample was properly sealed and transported, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any extra data entry not provided by an electronic chain-of-custody system.

Screening involves an initial rapid check for substances of abuse. While Screening is an economical method to rule out drug consumption on most samples, a positive screen must be corroborated to be acceptable in court. Any samples that are tentatively positive in Screening require a secondary confirmation.

If a sample shows a tentative positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are drawn from hair at a considerably lower concentration than in other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are tentatively positive are washed before confirmation if required. The entire laboratory process from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
