What it is, How it works

Hair captures a long-lasting record of alcohol and drug use by embedding biomarkers within the hair shaft as it grows. When collected near the scalp, hair provides a detection window of up to 3 months for alcohol and drugs. Hair is straightforward to gather, relatively tamper-proof, and convenient to transport.

A sample measuring 1.5 inches consisting of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, ideal for initial screening and verification. For EtG tests, additional panels, or tests exceeding 10 panels, 150mg of hair is advised. It is suggested to measure the sample using a jeweler's scale. If scalp hair is absent, a similar quantity of body hair can be collected. The term head hair pertains solely to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug testing lab procedure encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the phase where a sample is first handled and integrated into the lab’s tracking system. It includes ensuring the sample’s seal and shipment were correct, assigning a random LAN (Laboratory Accessioning Number), and inputting any details missing from an electronic chain of custody system.

Screening serves as an initial assessment to detect drug abuse. While it efficiently verifies the absence of drugs for most samples, any positive screening must be confirmed to be legally valid. Any samples deemed presumptively positive during Screening will need an additional confirmation step.

When a sample tests presumptive positive during Screening, additional hair from the initial sample is prepared for Extraction. This phase reduces drug concentration from the hair, which is a more challenging process compared to other methods like urine or oral fluid. Thus, hair drug testing is the most sophisticated methodology.

Confirmation of any positive result from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Prior to confirmation, all presumptive positive samples undergo washing as necessary. The end-to-end process from Accessioning through Confirmation is governed by both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
