What it is, How it works

Hair analysis is now regarded as an effective method for identifying drug and alcohol consumption. Hair captures a long-lasting record of alcohol and drug use by embedding biomarkers within the hair shaft as it grows. When collected near the scalp, hair provides a detection window of up to 3 months for alcohol and drugs. Hair is straightforward to gather, relatively tamper-proof, and convenient to transport.

A sample measuring 1.5 inches consisting of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, ideal for initial screening and verification. For EtG tests, additional panels, or tests exceeding 10 panels, 150mg of hair is advised. It is suggested to measure the sample using a jeweler's scale. If scalp hair is absent, a similar quantity of body hair can be collected. The term head hair pertains solely to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug testing lab procedure encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the phase where a sample is first handled and integrated into the lab’s tracking system. It includes ensuring the sample’s seal and shipment were correct, assigning a random LAN (Laboratory Accessioning Number), and inputting any details missing from an electronic chain of custody system.

Screening serves as an initial assessment to detect drug abuse. While it efficiently verifies the absence of drugs for most samples, any positive screening must be confirmed to be legally valid. Any samples deemed presumptively positive during Screening will need an additional confirmation step.

When a sample tests presumptive positive during Screening, additional hair from the initial sample is prepared for Extraction. This phase reduces drug concentration from the hair, which is a more challenging process compared to other methods like urine or oral fluid. Thus, hair drug testing is the most sophisticated methodology.

Confirmation of any positive result from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Prior to confirmation, all presumptive positive samples undergo washing as necessary. The end-to-end process from Accessioning through Confirmation is governed by both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests identify drug usage up to 90 days, unlike urine tests with shorter durations.
  • Resistance to tampering: Cheating a hair drug test is extremely hard, improving result accuracy.
  • Reveals usage patterns: It unveils a pattern of drug use over time, beyond only recent usage.

Limitations:

  • Not effective for recent consumption: Drugs become detectable in hair after roughly 5-7 days.
  • Higher costs: Hair drug tests generally cost more than alternative testing methods.
  • Result variations: Factors like hair color and individual hair growth differences can affect drug metabolite concentrations in hair.

Note: Although often termed as "hair follicle tests," the test actually analyzes the hair shaft, not the follicle beneath the scalp.
