Obtaining a 1.5-inch portion of about 200 hair strands (equivalent to a #2 pencil’s width) nearest the scalp will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, supplementary tests, and tests surpassing 10 panels, a 150mg sample is advisable. We advise measuring the sample using a jeweler's scale. In instances where scalp hair is unavailable, a corresponding amount of body hair can be gathered. Our reference to head hair exclusively means scalp hair. Body hair encompasses all other types like facial and axillary hair.

Process Overview

The laboratory procedure for processing a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially integrating the sample into the laboratory’s framework. This step ensures the sample was correctly sealed and sent, assigns a unique LAN (Laboratory Accessioning Number), and concludes any further data entry absent in an electronic chain of custody.

Screening serves as a preliminary assessment for substances of abuse. Despite being economical for dismissing usage in most cases, a positive result from Screening necessitates confirmation for legal acceptability. Any samples with presumptive positive findings in Screening require subsequent confirmation.

When a sample appears presumptively positive during Screening, additional hair is extracted from the original specimen for further preparation in Extraction. In this stage, the concentration of drugs drawn from the hair is significantly lower compared to other methods (e.g., urine or oral fluid), highlighting why hair testing is the most challenging methodology.

Confirmation for any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples with presumptive positives are cleansed before confirmation if required. The complete laboratory process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
