For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of the hair sample is suggested. Utilizing a jeweler’s scale for weighing is advisable. Should scalp hair be inaccessible, a corresponding amount of body hair might be used. "Head hair" specifically denotes scalp hair, whereas "Body hair" refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is comprised of four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating a sample into a lab’s processing system. Should a sample test presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (like urine or oral fluid), showcasing why hair drug screening is particularly challenging to execute.

Positive screening results must undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with presumptive positive findings are washed as required. The complete laboratory sequence from Accessioning to Confirmation is subject to review per the CAP (College of American Pathologists) Hair designation and certification under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
