What it is, How it works

Hair testing has emerged as an effective means for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drug use by embedding biomarkers within the fibers of the growing strand. If collected close to the scalp, hair can offer up to a 3-month detection window for alcohol and other substances. Hair is straightforward to gather, fairly challenging to tamper with, and easy to mail.

A segment of 1.5 inches comprising roughly 200 strands of hair (comparable to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and verification. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is suggested. We suggest using a jeweler’s scale to measure the specimen. If scalp hair isn't present, a similar quantity of body hair can be obtained. In the context of head hair, we imply scalp hair only. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The fundamental stages in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the primary processing of a sample into a lab’s system. This consists of confirming the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary quick assessment for substances of abuse. While Screening is a budget-friendly way to rule out drug usage in the vast majority of samples, a positive screen requires confirmation to be admissible in legal proceedings. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is taken from the original specimen and set for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than in other techniques (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to undertake.

Confirmation of any positive screen result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
