What it is, How it works

When collected near the scalp, hair can present a detection window of up to roughly three months for alcohol and other substances. Hair collection is straightforward, somewhat resistant to tampering, and can be transported with ease.

A sample measuring 1.5 inches, comprising about 200 hair strands (similar in size to a #2 pencil) nearest to the scalp, yields 100mg, an optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is advised. We suggest using a jeweler's scale for specimen weighing. If scalp hair is unavailable, an equivalent amount of body hair may be collected. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of drug test results consists of four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a laboratory's system. This step verifies that the sample has been correctly sealed and delivered, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Although Screening is a cost-effective strategy to rule out drug usage in most samples, any positive screen result requires confirmation to be court-admissible. Samples flagged as presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests presumptively positive at the Screening phase, more hair is extracted from the original specimen for preparation during the Extraction stage. In this phase, drugs are extracted from hair at much lower concentrations than other methods (such as urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Use of GC/MS, GC/MS/MS, or LC/MS/MS facilitates Confirmation of any positive screening results. Presumptive positive samples are washed before confirmation as necessary. The full laboratory process from Accessioning through Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
