What it is, How it works

When taken near the scalp, hair can reveal drug and alcohol use within a roughly three-month timeframe. Hair sample collection is straightforward, relatively resistant to tampering, and easy to transport.

A hair sample measuring 1.5 inches, consisting of around 200 hair strands (comparable to the thickness of a #2 pencil), will yield 100mg of hair, which is the ideal amount for screening and validation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. It's best to weigh the specimen with a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair can be obtained. Note that when we mention head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory process of obtaining a drug test result consists of four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is first processed into a lab's system. This step entails checking that the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any other required data entry not handled by an electronic chain of custody system.

Screening includes an initial rapid inspection for drugs of abuse. While Screening is a cost-efficient method to identify drug use for most samples, a screen showing positive requires confirmation to be used legally. Samples presumed positive during Screening necessitate further confirmation tests.

If Screening shows a sample is likely positive, more hair is extracted from the primary specimen and readied for Extraction. During this phase, drugs are separated from hair at much lower concentrations than other methods (such as urine or oral fluid), which makes hair drug testing the most challenging method to execute.

Confirming any positive screening tests is done using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation, if needed. The lab process ranging from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
