The process of collecting hair is straightforward, resistant to tampering, and convenient for shipping.

Collecting a 1.5-inch sample comprising roughly 200 strands closest to the scalp will yield 100mg of hair, ideal for both screening and confirmation tests. For additional tests such as EtG or those exceeding 10 panels, it is advised to gather 150mg of the sample. A jeweler’s scale is recommended for weighing the sample. In the absence of scalp hair, a similar quantity of body hair can be used. It should be noted that the term 'head hair' specifically pertains to scalp hair, while 'body hair' encompasses all other types, including facial and axillary hair.

Process Overview

The laboratory process for drug test analysis encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample integrates into the lab's system through initial processing. This entails ensuring the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and accomplishing any outstanding data entries not supplied by an electronic chain of custody.

The Screening phase provides an initial rapid assessment for drug presence. While cost-effective in ruling out drug use in many instances, any positive finding in Screening must undergo confirmation to stand up in court proceedings. Samples flagged as positive in Screening proceed to additional confirmation.

In cases where Screening suggests positivity, additional hair from the initial sample undergoes preparation for Extraction. Here, drugs are extracted at lower concentrations than those found in other test methods (e.g., urine or oral fluid), accounting for the challenges of hair drug screening.

Positive Screening results undergo Confirmation through methods like GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples receive a wash prior to confirmation, when needed. The entire lab process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
