The use of hair testing has gained recognition as an effective tool for identifying drug and alcohol consumption. Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the growing strand. When taken near the scalp, hair can offer up to roughly a 3-month detection period for substances such as alcohol and various drugs. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample consisting of around 200 strands of hair (approximately the thickness of a #2 pencil) closest to the scalp can yield 100mg of hair, which is deemed ideal for both initial screening and confirmation. For EtG, add-ons, and/or panels exceeding 10 tests, 150mg of hair is suggested. We advise using a jeweler’s scale to measure the specimen. If scalp hair isn't an option, a comparable quantity of body hair can be gathered. By head hair, we mean specifically scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s data system. This stage encompasses confirming the sample's sealing and proper shipping, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any other necessary data entries absent from an electronic chain of custody setup.

Screening serves as an initial swift assessment for drugs of abuse. Although Screening is a practical means to exclude drug use for most samples, a positive result needs verification to be court-admissible. Samples that return a presumptive positive in Screening necessitate secondary confirmation.

Should a sample test presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this scope, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting why hair drug testing is among the most challenging methodologies to execute.

Confirmation of any positive Screen outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing ahead of confirmation if needed. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair program and the accreditation concerning ISO / IEC 17025 standards.
