The collection of hair is straightforward, its tampering is relatively challenging, and it is uncomplicated to transport.

A segment measuring 1.5 inches of roughly 200 hair strands (comparable in size to a #2 pencil) situated nearest the scalp generates 100mg of hair, which is the perfect amount for initial tests and verifications. For EtG, additional tests, or panels exceeding ten, a 150mg sample is preferable. It is advised to use a jeweler’s scale for weighing the hair sample. If scalp hair is not accessible, an equivalent portion of body hair can be utilized. The term "head hair" specifically refers to scalp hair. In contrast, "body hair" includes all other types, such as facial or axillary hair.

Process Overview

The drug testing process in a laboratory consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where the sample is organized within the laboratory system. This phase includes confirming the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and entering any additional data not covered by electronic chain of custody protocols.

Screening provides a preliminary examination for drugs of abuse. Although economical for excluding drug use in most samples, a positive screen must undergo confirmation to be legally valid. Samples showing a provisional positive in Screening necessitate another confirmation phase.

In instances where a sample is preliminarily positive during Screening, additional hair is taken from the first specimen for Extraction. At this point, drugs are isolated from hair in significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair analysis a particularly complex testing method.

Any positive results from Screening are verified using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that register presumptively positive are washed pre-confirmation, if needed. The comprehensive lab process from Accessioning through to Confirmation undergoes scrutiny according to the CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.
