A sample measuring 1.5 inches of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp provides 100mg of hair, an ideal quantity for both screening and verification. For tests like EtG, add-ons, and those exceeding 10 panels, a 150mg sample of the hair is advised. It is suggested to measure the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair may be taken. References to head hair imply only scalp hair. Body hair includes any other hair types (facial, axillary, etc.).

Process Overview

The key steps in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initial sample processing within a lab’s system which includes confirming proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and finalizing any other data entry that an electronic chain of custody system might miss.

Screening is a preliminary and swift inspection for substances of abuse. It offers an economical method to eliminate drug usage from most samples, but a positive screening calls for confirmation to be legally valid. Presumptively positive samples from Screening must undergo a further confirmation.

In cases of presumptive positives in Screening, further hair is extracted from the initial amount for Extraction. Here, drugs are isolated from hair under lower concentration than other typical methods (e.g., urine or oral fluid), which renders hair analysis a challenging procedure.

Confirmation of any positive results from screening is executed through GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples are cleansed prior to confirmation if necessary. The entire lab process spanning Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Remarkable advancements in sample preparation, detection, and identification of drug compounds have been presented. The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Biomarkers and Pharmacokinetics: The identification and validation of reliable biomarkers and pharmacokinetic studies are fundamental aspects of drug testing and analysis. The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
