For EtG, additional tests, and more than ten panels, a 150mg sample is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't available, an equivalent portion of body hair may be taken. "Head hair" specifically means scalp hair, while "body hair" includes all other types, such as facial and axillary hair.

Process Overview

The laboratory processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes introducing a sample into the laboratory system by ensuring proper sealing and shipment, assigning a LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody.

Screening is a preliminary evaluation for drugs of abuse. Although it's a cost-efficient method to eliminate drug usage in most samples, a positive outcome requires confirmation for legal admissibility. Samples showing preliminary positive results in Screening are subject to further confirmation.

For samples showing preliminary positives in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or saliva). This makes hair drug screening one of the more challenging methodologies.

Confirming any positive result from screening employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumed positive samples are cleaned before confirmation as necessary. The complete laboratory procedure from Accessioning through Confirmation is assessed under both CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
