A hair sample of 1.5 inches, comprising about 200 strands (akin to the thickness of a #2 pencil) closest to the scalp, provides 100mg of hair, ideal for initial and confirmatory tests. For EtG, additional tests, and/or panels exceeding 10, a specimen weight of 150mg is suggested. It is advisable to weigh the specimen using a jeweler’s scale. In cases where scalp hair is not accessible, a comparable volume of body hair may be used. The term head hair refers exclusively to scalp hair. Body hair encompasses all other hair types (such as facial and axillary).

Process Overview

The four core steps in laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails initially processing a sample into a laboratory’s framework. This process involves ensuring the sample's proper sealing and shipment, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any manual data entry not provided by an electronic chain of custody system.

Screening conducts an initial swift evaluation for substances of abuse. While Screening is an economical means to rule out drug use for the majority of samples, a positive result necessitates confirmation to be valid in legal scenarios. Samples presumed positive during Screening require secondary verification.

If a sample is deemed presumptively positive during Screening, additional hair is extracted from the original specimen for preparation in the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other methodologies (for example, urine or oral fluid), which is why hair drug testing is notably complex.

Positive screenings are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are washed prior to confirmation as appropriate. The full lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
