A 1.5-inch section of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, the optimal sample size for analysis and validation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair can be gathered. "Head hair" refers narrowly to scalp hair alone. "Body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into a lab's system. This encompasses confirming the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not furnished by an electronic chain of custody system.

Screening involves a preliminary swift check for drug abuse. Though Screening is a cost-efficient method to rule out drug use in most samples, a positive screen necessitates confirmation for legality in court. Samples that screen as presumptively positive require further verification.

If a sample tests presumptively positive in Screening, additional hair is sourced from the original specimen and prepped for Extraction. During this stage, drugs are extracted from hair at considerably lower concentrations than other techniques (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS methods. All samples showing presumptive positivity are cleaned before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
