A 1.5-inch segment consisting of approximately 200 hairs (similar in thickness to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the preferred sample size for testing and verification. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advisable. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair is absent, an equivalent volume of body hair can be utilized. When mentioning head hair, we refer exclusively to scalp hair. Body hair pertains to all other types (such as facial, axillary, etc.).

Process Overview

Four primary stages are involved in processing a drug test result in the laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into the laboratory's system. This step verifies the sample was sealed and sent correctly, assigns a LAN (Laboratory Accessioning Number), and fills in any data missing from an electronic chain of custody system.

Screening performs a preliminary inspection for drugs of abuse. While it is a cost-efficient method for dismissing drug consumption in most samples, a positive result during Screening requires confirmation to be court-admissible. Any sample showing a presumptive positive in Screening requires secondary validation.

With a presumptive positive finding in Screening, more hair is retrieved from the initial sample for Extraction. Here, drugs are isolated from hair at much lower concentrations than with other methods (like urine or oral fluid), which is why hair drug screening presents the greatest challenge in execution.

Confirming any positive screening result is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before confirmation as necessary. From Accessioning to Confirmation, the entire laboratory process is subjected to review under both CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
