A 1.5-inch length of about 200 hair strands (similar to a #2 pencil's width) nearest the scalp yields 100mg of hair, the optimal sample for screening and confirmation. For tests like EtG, add-ons, and those exceeding 10 panels, a 150mg sample is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be collected. When discussing head hair, we refer exclusively to scalp hair. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The primary phases in laboratory analysis for drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial processing of a sample into a lab’s system, which covers verifying the sample's sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not handled by an electronic chain of custody system.

Screening involves a preliminary quick drug abuse check. Although Screening is a cost-efficient method for eliminating drug usage in most samples, a positive outcome must be confirmed for court admissibility. Any samples that show presumptive positive results in Screening must undergo a subsequent confirmation.

If a sample shows presumptive positivity in the Screening stage, additional hair is taken from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at a much lower concentration than with other methodologies (e.g., urine or oral fluid), highlighting why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive Screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are washed before confirmation as required. The complete laboratory workflow from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
